As a locally owned and operated hospice care provider in the Los Angeles Area, Hospice Care Partners emerged in 2021 in Sherman Oaks, California, with a clear vision of delivering exceptional hospice care to our neighboring communities.
Today, we have relocated to a more central position, enabling us to serve the entire Los Angeles community with even greater efficacy. We take immense pride in our accreditation by ACHC, which attests to our unwavering dedication to maintaining exemplary standards of practice for Home Care and Hospice services.

What We Do
Fountain Hospice is dedicated to protecting our patients and providing uninterrupted hospice care during the COVID-19 outbreak. We are available 24/7 including nights and weekends, wherever our patients call home (hospitals, skilled nursing facilities, assisted living facilities and private homes). We have implemented numerous efforts to ensure continuity of care and provide support to our patients and their families.
